Objectives: To investigate the association between breastfeeding practices and pain history at ages 7, 10, and 13. Methods: Prospective study involving 5,925 participants from the Generation XXI birth cohort in Porto, Portugal. Perinatal, sociodemographic, and breastfeeding (any and exclusive) characteristics were obtained by trained interviewers during recruitment (2005-06) and follow-up visits. Pain history was collected at ages 7,10, and 13 using the Luebeck pain screening questionnaire. Adjusted odds ratios (OR) and 95% confidence intervals (CI) were obtained from multinomial logistic regression and imputation techniques to estimate the associations between breastfeeding practices and pain features (frequency plus daily living restrictions). Results: Higher prevalences of pain were observed in children of primiparous mothers (60%) and female sex (51%). Between ages 7 and 13, prevalences of low-frequency pain (LFP) with no daily living restrictions (DLR) changed from 5% to 9%; medium/high-frequency pain (MHFP) with no (DLR) from 9% to 18%; LFP and MHFP with DLR from 5% to 3% and 9% to 6%, respectively. Children who were breastfed for ≥ 6 and
