Introduction: The safety cross system is a system of management of notifications for security of the patient, developed especially for health organizations that seek to become a safe care environment. Objective: to implement evidence with the Safety Cross System as an innovative tool for incident reporting at an Evidence Based Integrated Rehabilitation Center in the State of Pará. Methodology: This study consists of an implementation of evidence on an incident notification system that uses a cross calendar to record the events. It enables a visual methodology to manage patient safety in a transparent way, that is, the process records notifications of incidents with different colors for classification. Results are released daily and discussed between the Patient Safety Nucleus and the multiprofessional team. Result: The use of this system has become an important instrument for monitoring and demystifying notifications of incidents in the institution, increasing the registration number gradually between the month of December of the year prior to March 2019. Recorded December 0 (January), January 5 (five), February 9 (nine) and March 15. Thus, it is visualized that there was a team's adherence to the records, which is one of the patient's safety practices. Conclusion: The attractive functions of this system, the team feels more comfortable to be part of the notification and analysis process, proposing improvements to patient safety processes. The system is very practical and easy to handle, besides the acceptance of the assistance team.
