Introduction: Wounds are injuries that, when not properly treated, can cause complications for the patient. In Brazil, the large number of people affected by skin lesions represents, today, an important public health problem, with considerable morbidity. Effectiveness in treating wounds is directly related to the nurse's management and care skills. The implementation of the nursing consultation, in primary health care, for the treatment of wounds, allows the understanding of the care management adopted by the nurse, through the use of standardized languages, essential for a clinical judgment of the patient's needs, as well as for recognition of nursing interventions necessary to achieve nursing-sensitive expected outcomes. Objectives: To elaborate an instrument for the implementation of the nursing consultation, in the care of patients with complex wounds, in primary health care. Methods: This is a methodological study of technical production, developed by nursing students from a private institution in the interior of São Paulo, in partnership with the Municipal Health Department of Pindorama/SP, at the dressing clinic in a basic health unit in this state. County. The elaboration of the instrument was processed in two stages: 1) Theoretical-scientific foundation – in the databases Lilacs, Scielo, Google Scholar and Text Books; 2) Conception, construction and formatting of the instrument with the aid of the Microsoft Word program. Results: After the analysis and synthesis of the theoretical framework, the information was categorized and structured into: 1. Patient identification; 2. General assessment of the patient – cephalopodal physical examination; 3. Classification and assessment of the lesion; 4. Determination of pressure injury risk (Braden score), 5. Nursing diagnoses; 6. Nursing outcomes; 7. Nursing interventions; 8. Treatment/Toppings used; 9. Daily assessment record. The instrument has 11 pages. Conclusions: The instrument presents relevant information in the context of comprehensive care for patients with complex wounds in their biopsychosocial-spiritual aspect and its implementation will help in the process of standardizing nursing consultations in primary health care.
