Abstract

INTRODUCTION AND OBJECTIVES: Oral squamous cell carcinoma (OSCC) is one of the 10 most common types of cancer, with more than 300,000 new cases each year. Although progress has been made in cancer research and therapy, the survival rate has not improved significantly in recent years. Plants of the genus Piper are used in traditional medicine to treat cancer, and they present cytotoxic properties. MATERIAL AND METHODS: Methanol crude extract of Piper cernuum leaves (CEPCL) and hexane, dichloromethane and ethyl acetate chemical partitions were produced (HPPCL, APPCL, DPPCL). Chromatographic partitions were prepared from the dichloromethane chemical partition. Partitions and fractions were tested by clonogenic and cell viability (MTT), using carboplatin as positive control. OSCC cell lines and primary human fibroblasts were used for all assays. Toxicity tests were performed in mice according to the CEUA/UFF#982 protocol. The administration of carcinogen 4-Nitroquinoline 1-oxide (4NQO) was carried out in C57Bl/6 mice in the first 8 weeks after waiting for 8 weeks for tumor development that were confirmed visually. Subsequently, treatment for 8 weeks with intraperitoneal injection of preparation of partition FPC-D, 30mg/kg, twice a week. Then, macroscopic and histopathological analysis of the tongue of these animals was performed. Survival was analyzed by a Kaplan-Meier curve. RESULTS AND CONCLUSION: In the clonogenic test, it was observed that CEPCL significantly reduced the cell density of the SCC9 line at all concentrations tested and reduced cell viability when compared to control. Although all partitions showed cytotoxicity, the dichloromethane partition (DPPCL) was the most active. This partition induced reduction of cell number, induced membrane permeabilization (6.5 times more cells than control), was not hemolytic and not acutely toxic in mice. Seven chromatographic partitions of DPPCL were analyzed for its cytotoxicity where fractions 9 and 12 were the most active. Fractions 9 and 14 were partitioned again. Each fraction with 5 new partitions. All new fractions were tested and it was seen that 09.07 and 14.05 were the most selective (S=2.03 and SI=2.53 respectively). In the in vivo experiment, there was a statistical difference in the survival curve with p>0.03. Histological analysis of tumor progression revealed macroscopic and microscopic differences between 4NQO and 4NQO+PCDF groups in the number and stage of tumors, indicating a possible therapeutic activity of this partition. After analysis, it can be concluded that the extracts, partitions and fractions of P. cernuum were active against the cell lines of SCC9. In addition, chromatographic partitions of dichloromethane from P. cernuum tested on SCC9 cells had a significant inhibitory effect on the viability of oral squamous cells. The objectives will now be to determine which major substances are present in the most active and selective partitions and cell death pathway.

Na China, por exemplo, quase 30 das 60 espécies de Piper são utilizadas por povos indígenas no tratamento de diferentes tipos de câncer.

O grau de seletividade dos compostos pode ser expresso pelo valor do índice seletivo (SI). Assim, cada valor de SI para cada amostra testada foi calculado usando a seguinte fórmula: SI = IC50 célula normal / IC50 célula cancerosa. As amostras que apresentam SI superior a 1 são consideradas como tóxicas seletivamente para células cancerosas.

Na toxicidade foram avaliados alguns parâmetros, como: peso dos animais, análise visual de presença ou não de tumores, se ocorreram tremores, convulsão, salivação, diarreia, coma, sinais de dor, arqueamento das costas e dificuldade de mobilidade. Além disso, também foram realizadas análises histopatológicas dos seguintes órgãos: pulmões, coração, baço, fígado e rins.

Minha participação foi a realização de toda parte do ensaio in vitro com as células tumorais e as células normais, e colaborei com os ensaios in vivo realizados pelo aluno de mestrado Rafael Carriello

Olá Giselle, muito obrigada! Isso, o caminho é ir afunilando até que se chegue a uma substância ou substâncias ativas. Os ensaios in vitro e in vivo foram feitos de forma simultânea. Nós ainda não temos a substância ativa isolada da partição diclorometano de Piper cernuum para que possa ser calculada a concentração. Na parte in vivo, utilizamos o máximo de substância que poderia ser administrada sem que fosse tóxica para o animal.

Muito obrigada Israel! A viabilidade celular foi testada nas células sadias que foram os fibroblastos gengivais, e em linhagens tumorais de células de carcinoma de células escamosas oral (OSCC9).

A via de administração para indução de carcinogênese foi realizada através do carcinógeno 4-Nitroquinolina 1-óxido (4NQO) dissolvido em etilenoglicol e colocado na água de beber dos animais na concentração final de 100 μg/mL.

Já o tratamento com a fração diclorometano de Piper cernuum foi através de injeção intraperitonial com 48mg da partição dessa fração, após foi adicionado 1600μl de sulfóxido de dimetilo 100%, e 158,4 ml de solução salina tamponada com fosfato.

Oi Anneliese, muito obrigada!

- A metástase aparece somente após um ano e até que isso aconteça, ocorre muita mortalidade. Isso porque o tumor primário oral leva o animal a óbito antes da ocorrência da metástase. Então, seria necessário um "n" muito maior, que inviabiliza o desenho.

- Muito interessante a possibilidade de associação dos fármacos, e pensaremos nisso em um segundo momento! A intenção agora é avaliar se PCDF possui alguma função terapêutica isolada.

- Não foi medido o tamanho do tumor após o término do tratamento com PCDF, pois o tumor ocorre na língua do animal, e muitas vezes se torna inviável o acesso para medição e também por serem múltiplos nódulos.
